As home health providers rewire their operations to address value-based changes to the Medicare model, many overlook the primary indicator of care performance and value that the Case-Mix Index represents. Traditionally believed to reflect the accuracy of coding in the clinical program proposed at start of care, Home Health Case–Mix actually outlines the efficiency of the Medicare program as it relates to Chapter 7 of the Medicare Benefit Policy Manual. OASIS accuracy, required SOC documentation, global and proportional clinical programming, and a clear path to outcome achievement are all elements of care programming reflected in Case–Mix levels. In addition, Case–Mix achievement creates clinical programs that, if managed appropriately in real time, produce care episodes that achieve 4.5–5 level Star Ratings.
